We will match the personal information provided by you with personal information maintained by BlueStream. You will also have to submit a signed declaration under penalty of perjury that you are the consumer whose personal information is the subject of the request. 2. Right to Request Deletion of Personal Information

If you are an Authorized Agent acting on behalf of a consumer to exercise their right to opt-out, send an email request to [email protected]. You will be required to submit documentation as proof of the agency relationship.
